Grand Mother property transfer to grand sons name

We need to transfer the land document from my grand mother name to us i.e grand sons.
My grand mother has two properties one is which she purchased by her own and the other property is she got from her husband.
Can you please guide us how can we transfer my grand mother properties to our name
We are planing to write on my name and my brother name
My Grand mother, my father, mother are exipred

As per your submission it reveals that this is a matter of two independent properties
However
The property which is self aquired property of your grandmother it can be easily transfer through gift dee.
In so the property which seems to coparcenary property it required to be first partitioned as per Hindu succession Act

Majorly there are four certificates which are needed to be submitted, Death Certificate, Claim Application Form, Probate of will, Succession Certificate, moreover state laws could be applicable
