Call name of Mother recorded in Birth Certificate of Children (20 Years ago)

The call name of my wife has been recorded as Mother's Name in my Daughter and my Son's birth certificate. My daughter is 22 years old now and my son is 19 years old now. The call name is recorded in the School TC also. Seek your advice as the birth certificate will be required to be submitted when applying for Passport etc.

A.Yadi aap naam change karana chahate h. Tab aap ko competent officer se contact karna hoga. Yadi problems ka solution nhi nikalta h tab aap ko competent cout ke dwara naam change kara sakte hai.

A.In order to change your mother's name visit the municipal corporation and collect the correction form. Also ask the municipal corporation regarding the procedure for change of name. And you'll have to obtain an affidavit from local notary regarding change of name. And you'll have to make and advertisement in the local newspaper regarding change of name.

1. obtain the 'birth certificate/correction form' from the municipal corporation or the gram panchayat office where your children were born.
2. Obtain an affidavit from local notary and approach the officer regarding change of name in the birth certificate. Make an affidavit on a non-judicial stamp paper of minimal and mention the reason of name change.
3. Fill the correction form and submit it to the municipal corporation along with affidavit or certified copy with judge original signature.
4. After the completion of the application process, publish the advertisement in the local newspaper and your state official Gazette publication.

A.See try to go the Municipal corporation and ask them for a procedure to get modified birth certificate. If they say there is a possibility then do the same for the School TC.
In case it can not be done that way then the best way to handle this is that get a Fist Class Judicial Magistrate Affidavit done stating the fact that Call name had been mentioned in the Birth Certificate and TC and her Call name and real name (whatever) are one and same. It shall serve the purpose everywhere.
