A staff at our village-based project needs help with procuring BIRTH CERTIFICATES for his two daughters - who are 13 & 15 years old respectively.

Both of them were BORN AT HOME - and have NO PROOF OF BIRTH (Birth Certificate)

However, BOTH the girls DO HAVE AADHAR.

I am trying to help him procure Birth Certificates for his daughters - and found a website - https://edistrict.wb.gov.in/ - where such application can be registered.

The following DOCUMENT is required for the process - as per the INFORMATION available through the SUBMISSION FORM of the website:-
(1) Birth Proof = "Affidavit by petitioner"
(2) "Declaration from five local person for granting okay to delayed registration"

HOWEVER, THERE IS NO FURTHER INFORMATION OR DIRECTIVE AVAILABLE ON THE SITE!

MY QUERY:-

(A) WHAT SHOULD BE THE FORMAT FOR (1) AFFIDAVIT (2) DECLARATION ~ MENTIONED ABOVE?

(B) CAN WE GET SUCH AN AFFIDAVIT REGISTERED AT OUR LOCAL COURT [Under the Additional Chief Judicial Magistrate, Bolpur, Birbhum]?

(C) WHAT SHOULD THE THE APROX. LEGAL FEES AND OTHER COSTS FOR THE SAME?

A.Format will be available at lawyers office , you can make an affidavit and registered into local court, fees are flexible.
