Revert back my maiden name on Marriage certificate

I need my maiden name on marriage licence.They issued it as my first name then fathers surname then husband surname.i want tk change it back as my first name and last name please hlp me with procedure

A.You can get it changed by way of an affidavit at the marriage registrars office and then get it published in the official gazette.
You will be required to publish such change in two of the local news paper. One English and one regional.
