Form 5 is required for those who have completed a driving course at a recognized driving school and is essentially required when applying for a driving license to drive transport vehicles. A Driving Certificate is issued by the authorised motor training school or establishment in Form 5 under Rules 14(e), 17(1)(b), and 31A(2) of the CMV Rules, 1989, which is one of the other documents required for the issue of a driving license to the applicant to drive Transport Vehicle along with valid DL of Non-Transport Category of one year experience in the same category for which Transport Vehicle category is required by the licensing authority. Thus, Form 5 is already in place for the issue of a Driving License for driving of transport vehicle, and based on which the Licensing Authority issues driving licenses to the applicant. So, unless there is any deviation in practice or processing of an application for DL that apparently appears on the face of the record, further suggestions or advises are not essentially required. However, be informed that alone, incompetence to drive a vehicle is not the sole cause of a motor accident, which also includes negligence, rash driving, driving in a drunken condition, driving without valid DL or MV insurance, etc, are also equally liable for the cause of motor accident and based on the cause of motor accident, violator of traffic rules are prosecuted and fined even with imprisonment in some cased under the Motor Vehicles Act, 1988.
