Since you mistakenly declared Grade I fatty liver based on memory, but later found that the report actually related to your mother and your own report mentions a small liver polyp, you should write to the insurer clarifying that the earlier declaration was an inadvertent error, attach your correct medical report, and clearly state that you have not undergone any further liver investigations or treatment since then.

Request them to update the proposal form accordingly and obtain written confirmation of the corrected disclosure. Transparency at this stage protects you from future claim repudiation on grounds of misrepresentation or non-disclosure. Avoid verbal communication send everything by email and keep acknowledgment for record.
