Document to empower my mother to act on father's behalf

Hi, My father has Parkinson's disease for a few year now and for the last few months he has severe hallucinations and delusions and unable to make any coherent decisions (Medical and financial), he can recognise family members but not aware of current state of affairs. I wanted to take legal opinion on how my mother can have a power of attorney or similar document/power in order to manage and make decisions on my father's medical and financial affairs(pension, bank accounts, etc) .
One of the advocates mentioned, she can act in representative capacity. What does that mean? Is it a legal document? How can she apply for one? Approx time taken to get this? Approx cost associated?

It is better to take special or general power of attorney by your mother from your father and thus operate all the transactions of your father. Better such Power of Attorney is registered in the office of Sub-Registrar...
