You can change your surname back to you maiden surname by way of Affidavit before the first-class magistrate stating the facts.
along with documents i.e your Divorce papers, birth certificate for proof.

with the Affidavit, you can change your name in Pan, Aadhar etc.

A.Hello client,
Ma'am from your query the thing I got to know is that only in your office your name has been changed with your second husband's surname. Well if that's the only issue you can simply apply before your office authority and apply to change for the said. And in case if your documents are in a different surname . Then you can make an Affidavit by stating the same before a first class magistrate. You can publish the same in a newspaper as a notice.
Thank you.
