How annexure of book publishing MOA will be? What details it should have? How it will look like? Is it to take print out on A4 sheet and sign by both parties, publisher and Author enough ?

A.Hii,
The MOA should contain the following details:
Details of the parties, delivery of manuscript, advertisement, description of work, grant of rights, copyright information, payment, termination, signature of the parties.
Yes, the signature in the normal A4 sheet print out is enough.

A.Hi,
The MOA should contain all the relevant details such as royalty, delivery of manuscript, rights granted, copyright information, territorial information, compensation and money paid in advance, disputes, termination of contract, cost of publishing among other details. These are not exhaustive, but suggestive.
Yes you can take a print out in A4 sheet and get it signed by both parties, that would be enough.
