A.Dear Client,
Do you mean surname? what you are calling initial?
Two things are important.
1.You must tell the name of the father and Mother, date of birth, and other details correctly.Once the Birth Certificate is issued it is difficult to change. even amend.
2.You can give the name of the Child without surname.
3.If you are a single woman and do not have a husband or your husband does not want to come in to picture at this stage the you simply tell that you are single woman and do not want to give the name of the father of the child. No body will force you. Simply the Child will acquire your surname.
