Non Issuing of NDC by bank despite full principal and interest Non Issuing of NDC by bank despite full principal and interest

Sir, I had paid all EMIs with interest to bank five years ago of my personal loan. The bank has not closed my account till date saying that huge amount is outstanding due to penalty charges. My first EMI was dishonoured due to insufficient fund and I paid the same to bank collection agent in the same month. After that 14 more EMIs were dishonoured due to technical problem by my bank reg. e-mandate. I submitted ECS Mendate twice to my lending bank. I paid all EMIs in the same months. I also instructed the collection agent to collect the EMIs 10 days before my due date of EMI but he told that they would mandatorily get my EMIs bounced then only he can collect the EMI from me. I always paid my EMIs by cheque before the due date. The lender bank never sent me any email, written communication, legal notice, demand notice so far and now they started demanding huge amount as penalty, overdue charges, collection agent expenses etc. and they imposed interest @ 3.9% per month on my outstanding penal charges which is increasing every month. I sent several emails to the Owner, Country Head, Nodal Officer, Regional Head etc. and written representation to Bank's Regional Office in Delhi. But their telecallers r threatening me to face legal consequences and nobody is reply in writing. What should I do sir.
