lease deed/deficient service

1 lease deed was created between society and apartment at gultekdi office in pune.it is not visible on esearch website.i have docu no abd date is 23/10/1999..anyway to get tge deed.
2 govt hospital in pune took 1 year to do diagnosis of neurogical symptoms.i have docu 1 year back and now to show symptom has been noted one year back but test was advised after 1 year and it revealed abnormality.
3 state bank of india has not issued cheque book and atm card even after 25 days.icici bank has not responded to request that internet password is disabled even after 10 days.i have evidence of email sent.can compensation be claimed from govt hospital and these banks.

Talk to me personally to get best advice on the same matter and I shall help you with the same.
It is very hard to get any compensation from Government Hospital and Banks unless you may grounds around Civil Courts or Consumer Forums for years together.
