Fake documents created and house shown as open plot and registered..

My father bought 150 yards land on his mother's name in1980's, same year he constructed house in 150sq yards. After completion, We are staying in it from that day and paying municipal tax, current bill everything. Recently my dad registered that house on my mom's name to take bank loan for to construct +1 stair. And applied for mutation in municipal office. After 4months my grandmother divided our house as 75 sq yards, showing as open plot in plan and did gift registered to daughter, who has gone under adopt to other parents. My query is How registrar registered a house as open plot without looking it physically. The local municipal office object us and not giving permission to construct +1 stair permission & name mutation. Please provide your valuable suggestion. How to make that registration cancel. And how to get +1 stair permission and name mutation
