I bought lic policy in 2002 with name s.vasanthalakshmi I gave 12 the marksheet as proof after that i kept my name as s.vasanthi this name is my aadhaar card,voter id,pan card ,passport ,bank account ,ration card but now i have to change my name in lic if i didnt change in future it will create a problem so i didnt have any proof for change name s.vasantalakshmi to s.vasanthi .so can you give suggestion for easy process for name change.

A.Hi,
You may visit the nearest Branch and kindly carry the following documents:
1.Request Letter/Policy Servicing Form
2.Standard ID proof with DOB & correct name

1.Request Letter/Policy Servicing Form (available at branch)
2.Standard ID proof with DOB & correct name
3.Marriage certificate (If name change due to marriage)
4.Gazette notification (In case of complete name change)
5.Standard ID proof of Proposer
