A.Hi,
Since, your birth certificate must be stating your name as 'Deb' instead of 'Dey', it is advisable for you to get an affidavit declaring that you changed your name from Deelip Kumar Deb to Deelip Kumar Dey before 10th std.

A.Hi,
You can provide an affidavit stating that you (your full name) is the son of (your father's and mother's name).
The other option is you can get an affidavit declaring that you changed your name from Deelip Kumar Deb to Deelip Kumar Dey. This will solve the problem of your birth certificate having 'Deb' instead of 'Dey'.
