Name different in PF account and Hospital records Name different in PF account and Hospital records

I am writing for my cousin father. He have a name in certificate and same is mentioned in PF account. Later he have changed name and got new name in aadhar and also in Hospital records.

He have got affected with COVID 19 and and recently passed away. In order to raise claim settlement or insurance there will be discrepancy in death certificate/ Hospital records.

I request you to suggest the process to be followed so that PF claims and insurance claims are smooth.

A.Plz submitte his name change proof and related documents in insurense office
Then raise the claim procedure...

A.Hi,
You may use both names, with one serving as an alias. In addition, the legal heir should sign an affidavit stating that all of the names and records are correct. Submit these forms to the insurance provider, along with the name change paperwork.

A.Hi,
You can mention both the names, one as an alias. Also, the legal heir should make an affidavit declaring the names and all the relevant documents to be correct. Submit these documents as well as the papers of name change also to the insurance company.
