Sir/madam, My daughter is 8+yrs and her DOB is 30.10.2012 as per Birth certificate issued by Gram Panchayet. I am an employee of PSU and my daughter was born on 07.09.2012 at our Departmental Hospital . So, her DOB was recorded in my service profile as 07.09.2012. Now, my dept. is not accepting the DOB issued by Gram Panchayet because they have the Departmental hospital record. I approached 2~3 times to the Panchayat with hospital record for correction but every time they denied saying that DOB can not be corrected.
Recently, i discover that the application form filled at the time of applying DOB is in my wife's handwriting but the date of birth portion is not in her handwriting. As per senior clerk of that panchayet(unofficially told me) that...probably the middle man who engaged on that work, put the birth date as per their suitable date to maintain their record serially. because i applied for DOB after 1 month or little more.
So, what should i do now? AS PER WEST BENGAL STATE JUDICIARY SYSTEM.

A.Dear Sir,
There is a separate procedure as follows:

13. Delayed registration of births and deaths (1) Any birth or death of which information is given to the Registrar after the expiry of the period specified there for, but within thirty days of its occurrence, shall be registered on payment of such late fee as may be prescribed. (2) Any birth or death of which delayed information is given to the Registrar after thirty days but within one year of its occurrence shall be registered only with the written permission of the prescribed authority and on payment of the prescribed fee and the production of an affidavit made before a notary public or any other officer authorised in this behalf by the State Government. (3) Any birth or death which has not been registered within one year of its occurrence, shall be registered only on an order made by a Magistrate of the first class or a Presidency Magistrate after verifying the correctness of the birth or death and on payment of the prescribed fee. (4) The provisions of this section shall be without prejudice to any action that may be taken against a person for failure on his part to register any birth or death within the time specified therefor and any such birth or death may be registered during the pendency of any such action.

A.Get a first class JM affidavit done then apply to the gram panchayat or municipality for correction or re issuance of new birth certificate with corrected date of birth. That way you can get this done.

A.Hi,
You can file an affidavit of change and verification with a knowledgeable lawyer and have it registered in a court of law, as well as publishing the change of name in the newspaper.

A.Hi,
You can make an affidavit of change and verification which will be file by a learned lawyer and registered in the court of law and also publishing in the newspaper the change of name that you made.
