A.Dear Sir,
You must swear an affidavit saying that he is known by both the names. Better get it notarized and file before the concerned authority.

A.Dear Client,
To resolve the issue related to the name of your deceased father Y which appears differently in different documents, you need to pursue the following steps. 1) An affidavit is a legal document that is used to state facts under oath. The affidavit should be sworn by X either before the Notary Public or preferably before the Judicial Magistrate/Executive Magistrate, declaring that the 'A B C ' and the 'A C' are the same and one identical person. 2) The next step in the name change process is to publish a notification in the Gazette of India. The Gazette of India is a publication of the Government of India that contains official government notifications, including name change notifications. The notification should be in the prescribed format and published in at least two local newspapers. 3) After publishing the notification in the Gazette of India, the next step is to make out a name change deed. The name change deed is a legal document that confirms the name change and includes the details of the old and new names. The deed should be made on a stamp paper of the appropriate value as per the state where the applicant resides and should be signed by two witnesses and notarized by a Notary Public. 5) The final step is to submit the name change deed to the relevant authorities for information and records. The applicant may also need to provide additional documents, such as a copy of the Gazette notification and a copy of the affidavit for updating the records. It is essential to follow the correct procedure to ensure that the name change is recognized by the Government and other relevant authorities.

A.Dear Client,
Person X should draft an affidavit stating the relationship between themselves and their father, explaining the discrepancy in names ("A B C" and "A C"), and providing any additional relevant information. Enclose copies of the documents, such as birth certificates, school records, or any other official records that can confirm the relationship between person X and their father. The affidavit should be signed in front of a notary public. It is advisable to publish a notice in a local newspaper regarding discrepancies in name. This is done to inform the public and to prevent any legal issues in the future.
