You may approach High Court and hire a Senior Advocate to see that you get an order from the High Court to issue passport on your new address as nobody is suppose to stay in the same address.

File a name change form, attach a copy of the marriage certificate and pay the fee as prescribed for the procedure. Once the same is done 4 copies of the gazette carrying your application details and changed name will be mailed to the address mentioned in the application.

As per the facts mentioned, you can send an email to the concerned authority stating your mistake and get it rectified and if you don't get positive feedback then you can send a legal notice to get it rectified.

You should go to the concerned Gazette Office along with the printing matter. The officials will let you know on how to proceed further and what fees has to be paid. The same can be done through the online web portal called "e-gazette".
