10th marsheet my name is X Y Z but in 12th name is X Z. And my father name in 10th A Y and in 12th A Y Z what to do. Sir. Is affidavite work??
File a name change form, attach a copy of the marriage certificate and pay the fee as prescribed for the procedure. Once the same is done 4 copies of the gazette carrying your application details and changed name will be mailed to the address mentioned in the application.

5) AFFIDAVIT : ( If Any Made for any reasons like Numerology, Astrology, Marriage, Passport, Adoptions etc )
6) APPLICATION FORM : Duly Signed Application Form with OLD & NEW SIGNATURE other details like Old & New Name, NEW Correspondence address, Contact Details & reason for Name change
7) CASTE CERTIFICATE (IF ANY) : SC/ST/NT1,2,3
8) LEGAL DOCUMENTS (ANY ONE OR AS APPLICABLE) : Affidavit / Marriage Certificate/ Nikah- nama / Adoption Certificate/ Divorce papers/ Divorce Decree etc.
9) MOBILE NO. : For OTP
10) EMAIL ID : For LOGIN & GAZETTE COPY

Newspaper Publication.
Name Change Deed.
Request letter to the Secretariat.
Name Change Statement.
Reply Letter from the Secretariat.
Publication Money Receipt.
Gazette Notification.

Either you can get the names changed in the marksheet by approaching the board or university or you can get a first class magistrate affidavit done to show case that all the names are same and belong to you and your father.
