Hi, there is spelling mistake in my birth certificate, and school marksheet and i want to apply for the passport what all documents will i have to submit?

You are required to first get your name changed in the birth certificate and school mark sheet and you may approach the judicial magistrate of the first class to do the same. Then, the required annexure forms can be submitted as asked.

A.Submit all the documents along with a First Class Judicial Magistrate affidavit in support of these documents mentioning that all those names are one and the same and belong to you. You should also contact the passport department if they any particular format of affidavit for the same.
