Ladies Death certificate is entered with her Father's name instead of her Husband's name. what is procedure to change it with her Husband's name.

A.Dear Client,

Death Certificate is issued by the Registrar under the Registration of Birth and Death Act, 1969. Civic Authority/Municipality/Panchayat based on the documents required under the Act. The documents required under RBD Act in a State for issuing a death certificate of an individual are as follows: Death Certificate issued by the Hospital or a registered medical practitioner, Proof relating to the birth and age of the deceased - e.g, the birth certificate, matriculation certificate, PAN Card, AADHAAR Card, Voter ID Card, etc. and if required, an affidavit specifying the date and time of death sworn by the surviving legal heir of the deceased. So, you may apply to the concerned authority as stated above for the desired correction in the Death Certificate of the deceased attaching therewith all the relevant documents that support your claim for correction in the Death Certificate or as required by the concerned authority.

For example, in Karnataka they have Karnataka One Centre (KOC)
Collect the 'Death certificate updation/correction' form from the KOC and fill it completely. After filling the form, attach the documents mentioned in the required documents section below with the completed application form and then submit it to the KOC officer
