How to draft affidavit

I want to change my father's name in passport. I understand that my father has to get a affidavit done and then declare the name change in local newspaper. However, the name in all his documents are correct. Only my passport has my father's name recorded incorrectly. In this case how to draft the affidavit. Usually the affidavit mentions the old name recorded in so and so document and then the correct name to be changed. Please help me on the right way to draft the name change affidavit

Go to you local Notary and make an affidavit regarding change of your name. You have to mention therein your specific reason for changing your name.
It is necessary to have two witnesses sign the affidavit after printing it on simple stamp paper. You must get it approved by two officers of the rank of a gazetted officer to meet the requirements of the law.
