A.Dear Client,
Until and unless your biological parents change their surname by adopting the surname of their biological parents, you may not be able to change the surname held by your grandfather. Because all of your statutory records are made on the basis of the surname of your biological parents. However, To resolve the issue related to the surname of your biological parents that differs from the surname of your grandfather, you need to pursue the following steps. 1) An affidavit is a legal document that is used to state facts under oath. The affidavit should include the applicant's current name, the desired new name, the reason for the surname change, and any other relevant details declaring that the surname bearing "Srivastava' and the 'Mandal' are the same and one identical person. The affidavit should be made on a stamp paper of the appropriate value as per the state where the applicant resides and should be sworn either before the Notary Public or preferably before the Judicial Magistrate/Executive Magistrate. 2) The next step in the name change process is to publish a notification in the Gazette of India. The Gazette of India is a publication of the Government of India that contains official government notifications, including name change notifications. The notification should be in the prescribed format and published in at least two local newspapers. 3) After publishing the notification in the Gazette of India, the next step is to make out a surname change deed. The surname change deed is a legal document that confirms the name change and includes the details of the old and new surnames. The deed should be made on a stamp paper of the appropriate value as per the state where the applicant resides and should be signed by two witnesses and notarized by a Notary Public. 5) The final step is to submit the surname change deed to the relevant authorities for information and records. The applicant may also need to provide additional documents, such as a copy of the Gazette notification and a copy of the affidavit for updating the records. It is essential to follow the correct procedure to ensure that the name change is recognized by the Government and other relevant authorities.

A.Dear Client,
Prepare an affidavit stating your intention to change your surname from Shrivastava to Mondal. The affidavit should include your full name, current address, father's name, grandfather's name, and the reason for the name change.. The affidavit should be on non-judicial stamp paper of the requisite value and must be signed by you (the person changing the surname) in the presence of a notary public. Publish a notice in a local newspaper and in the Official Gazette of your state, declaring the change of your surname.
