Adding surname

My daughter born in uae .we have birth certificate issued by uae government and citizen letter from indian embassy (indian embassy letter same as birth certificate ).
While applying birth certificate we made a mistake by not adding surname ,now we want to add surname kindly suggest what is the procedure to add surname and get new certificate .
(Note:Now we settled in india not in uae )
