Final Report (FR) or Closure Report

For filing the Final Report (FR) or Closure Report by IO, does it needs to be approved by ACP or DCP, or IO is enough ?
And then the police will send this FR to Court also?
How tough is its approval from Court ?
A.Dear Sir,
If the offences are punishable with more than three years then such Closure Report is to be approved by higher police officers and a copy will be sent to Court for its approval also.
