How to get OBC CERTIFICATE with Affidavit

I belonging chasi kaibartta sub caste but I have only court Affidavit. How I proof my caste

The Caste certificate in India can be taken online or from the Revenue Department's local office or the SDM office. A local inquiry is conducted when the caste certificate is issued for the first time to a particular family.
Documents which are required for these are
1. Address
2. Copy of Ration Card / Copy of Voter Card / Name in the Voter List (one of them)
3. A report regarding caste Patwari / Sarpanch
4. Income report
5. Residence Proof
6. Caste / religion report
7. In case of Women caste certificate before marriage
8. Self-declaration Certificate that the above-given information is
correct.
After this if all the information is correct and got approved then you will get your caste certificate.
