A discrepancy of names on documents A discrepancy of names on documents

My name is a single word, i.e. I don't have a surname or a last name, and it is so on all my academic and identification documents. But now, I have done my Masters from the University of Mumbai, and they decided to add my Mother's name to my name on my mark sheets, creating a discrepancy of names on my documents. Would a same-name affidavit work in this case?

A.Dear Client,
A same-name affidavit is a legal document that can be used to confirm that you use a single-word name without a surname or last name. In your situation, where your academic and identification documents originally did not include your mother's name, but your University of Mumbai mark sheets now include it, a same-name affidavit may be helpful in reconciling the discrepancy.

Here's how you can use a same-name affidavit to address this issue:

Drafting the Affidavit: You should draft a same-name affidavit that clearly states your full name, including the single word you use as your name and your mother's name. The affidavit should explain that you have been using a single-word name without a surname for all your academic and identification documents.

Notarization: Once the affidavit is drafted, you should visit a notary public to have it notarized. Notarization involves the notary public verifying your identity and witnessing your signature on the affidavit.

Submit to Authorities: After notarization, you can submit copies of the same-name affidavit to the relevant authorities, such as the University of Mumbai or any other institutions or agencies that have included your mother's name on your documents. Include a formal request to correct the documents to reflect your preferred single-word name without a surname.

Follow Up: Follow up with the authorities to ensure that they process your request for correction and update your documents accordingly.

Keep Copies: Always retain copies of the same-name affidavit and any correspondence related to the correction process for your records.

A same-name affidavit can serve as legal documentation of your preferred name and help address discrepancies in your documents.
Thank you.
