1. we got mutual consent 2018, and no where mentioned in the order that my ex has no rights to visit my child and no custody
2.He had filed G and W Act after that he closed when I closed the divorce case first. Does this mean he has rights to file new case for custody or visiting?
3.I need to make a passport for my kid and do I need his consent? without consent may I proceed? will it lead to issues?

Since the question of child custody is not finalized, so either you have to get NOC either from your husband or file a permanent child custody case and get interim order to take the child outside India.
