A.dear client in your matter you can appeal before the Court for a medical examination and also if you have documents to prove like medical reports or prescriptions or diagnosis etc.
You may take a consultation for further Legal Advice.

A.You can file an o.p for divorce in the family court showing the reason of mental disoder and if your husband objects the petition you can file a petition for medical examination of your husband before medical board. For that purpose you have to produce medical treament records, bills, discharge summary, lab report,o.p.card etc before the court to prove the same

A.Take him to a Doctor or counselor such documents given by a doctor is enough for filing divorce case. also if you cannot do so then file a case and plead before the court to medically examine your husband.
