You may ask the bank to lodge a complaint with police about misplacement of documents and such endorsement may be used by as secondary evidence along with certified/duplicate copies of your originals.

A. Talk to the bank authorities and let them know that you have an intention to pay the EMI and shall pay it. So that they do not initiate any sort of proceedings against you.

You need not worry. If you have address of such party then file a suit for permanent injunction and restrain him permanently from making such threatening calls to you etc.

Yes after the death of the guaranter an Application for substitution of legal heirs have to be filed to bring on record the legal heirs of the guaranter and therefater they can contest the same. If the bank has done fraud you should file a Criminal Complaint against it immediately.
