Since your mother died last year and aadhar has been there for such a long time then how come that there is no aadhar or voter id in your mother's name? This is quite surprising. In this situation you should get a first class Judicial Magistrate affidavit done to move ahead if the hospital is fine to accept that.

You can send an application to the authorities who issued the 10th mark sheet in the first place and get the DOB changed.
If they don’t accept your application then you may get an order from the court directing the authorities to update the correct DOB.

File a name change form, attach a copy of the marriage certificate and pay the fee as prescribed for the procedure. Once the same is done 4 copies of the gazette carrying your application details and changed name will be mailed to the address mentioned in the application.

An unregistered PoA is legally valid but for future safety, it is advisable to get it registered after a notary. An e-sign is also legally valid as per Schedule II, IT Act but again registration is advised in each case.
